Privacy Policy: Our Commitment to Protecting Your Information

This Privacy Policy explains how Connect Hearing, Inc.  (“Connect Hearing”, “we”, “our”, or “us”) collects, uses, and discloses information we obtain through or any other service, application or product[A1]  that links to this Privacy Policy (collectively, the “Service”).  This Privacy Policy is incorporated by reference into Connect Hearing’s Terms & Conditions (the “Terms”).  By using the Service, you consent to the processing of your information as set forth in this Privacy Policy. 

All of your protected health information maintained by Connect Hearing, including information you provide on the Service, is also subject to the Notice of Privacy Practices issued by Connect Hearing under the Health Insurance Portability and Accountability Act (“HIPAA”).  In addition, the information you submit to request an appointment through the Service is also subject to our Consumer Communication Notice.  The Notice of Privacy Practices and Consumer Communications Notice contain additional provisions relating to the use and disclosure of your protected health information that go beyond the terms of this Privacy Policy.

How We Collect Your Information

Information You Provide

We collect any information that you provide us when you use the Service, including when you request an appointment, take an online hearing test, signup for our newsletter, interact with our customer-support team, or otherwise access the Service.  For example, when you request an appointment through the Service we collect your first and last name, phone number, and email address.  

Information We Collect When You Use Our Service

We may automatically collect certain technical information from your computer or mobile device when you visit the Service.

Device Information. We receive information from your device, including IP address, web browser type, mobile operating system version, and unique device identifiers.  We may also collect and store your browser settings and preferences.

Usage Information. We receive information about your interactions with the Service, such as your interaction with certain features, the time spent on certain pages, the dates and times of your visits, and, if available, the address of the website you linked from.

Service Providers

We may share your information among our vendors, service providers, and other third parties that perform services on our behalf.  In addition, we may use third party analytics vendors to evaluate and provide us with information about your use of the Service. These analytics service providers may set and access their own cookies, pixel tags and similar technologies on the Service and they may otherwise collect or have access to information about you which they may collect over time and across different websites.

As Required by Law and Similar Disclosures

Ad Network Providers

When you visit our Service, ad network providers may set and access their own cookies, pixel tags and similar technologies on your device and they may otherwise collect or have access to information about you. We and our ad network providers may target advertisements for products and services in which you might be interested based on your use of the Service.

If you prefer to not receive targeted advertising, you can opt-out of some network advertising programs that use your information. Social Media Companies

Our Service includes social media features, such as the Facebook button. When you visit our Service and other Service’s that include such features, they may collect your IP address, which page you are visiting on our site, and may set a cookie to enable the feature to function properly. Social media features are either hosted by a third party or hosted directly on our Service. Your interactions with these features are governed by the privacy statement of the company providing them.

Merger, Sale, or Other Asset Transfers

Our Service includes links to other websites whose privacy practices may differ from those of Connect Hearing. If you submit personal information to any of those sites, your information is governed by their privacy statements. We encourage you to carefully read the privacy statement of any website you visit.

How We Hold & Protect Your Information

Unfortunately, the Internet cannot be guaranteed to be 100% secure, and we cannot ensure or warrant the security of any information you provide to us. Children's Privacy

We do not knowingly collect personally identifiable information from children under the age of 13. If Connect Hearing is made aware that it has collected personal information from a child under 13, we will take steps to delete this information.

International Users

If you are visiting from the European Economic Area or other regions with laws governing data collection and use, please note that we may transfer your information to recipients in countries other than the country in which the information was originally collected, including the United States of America. Those countries may not have the same data protection laws as the country in which you initially provided the information. Changes to our Privacy Policy & Practices

This Privacy Policy may be updated periodically and without prior notice to you to reflect changes in our information practices or relevant laws. To inform you of any change, we will post the updated Privacy Policy on this page and indicate at the bottom of this Privacy Policy when it was updated.
